Health and Care Transformation in South West London
The six boroughs are working together to help transform the health and social care system in South West London so it is able to meet future needs and challenges – a growing and ageing population with greater numbers of individuals with complex care needs, supporting people to live in their own homes and communities for as long as possible, adoption of new approaches, skills and technology within the context of increasing financial pressure on public funding.
The SLP team support the boroughs in working with health partners across South West London to shape and implement the South West London Sustainability and Transformation Plan (SWL STP). The SWL STP was published in October 2016 and sets out strategic plans to transform health and care services in South West London by 2021 to meet the growing and changing needs of the population in a financially sustainable way.
